Digital re-prescription service
This is the digital re-prescription request service for Lucena Clinic CAMHS.
Service users and families use this to request a repeat prescription between appointments and medication reviews. To request a repeat will you need the following:
1) The location of the Lucena Team you attend
2) Your child’s MRN. This number is on the top right of the appointment letters you receive.
3) The young person’s date of birth
All of these need to be entered correctly or we will not be able to issue your repeat prescription. All information you put into this form will be shared with the relevant staff and deleted once the repeat prescription has been sent.
